Our assessments cover multiple areas including functional status, social structure, health, nursing/medical, cognitive screening, a basic review of financial resources, as well as home safety evaluations. This holistic approach allows us to have a complete understanding of each client’s situation and can develop the best plan to meet their needs.
We create a personalized care plan for each client that incorporates recommendations related to medical and healthcare issues, senior community resources, housing options, referrals for financial and legal planning, and more. The care plan is designed with both short-term and long-term goals in mind. We also take steps to execute the care plan recommendations as requested and providing ongoing support to meet our client’s evolving needs.

We arrange and attend doctor appointments, therapy sessions, or medical procedures on behalf of our clients, to make certiain their needs are met throughout the process. Additionally, we monitor our client’s adherence to medical orders and instructions, while coordinating and overseeing the work of service providers to ensure consistency and quality of care.

We offer crisis management and advocacy services for our clients in a variety of settings, including at home, in hospitals, nursing homes, and assisted living communities. Our role is to act as a dedicated advocate, so that clients’ needs are addressed promptly, their rights are upheld, and they receive the appropriate care in times of a health care crisis. Whether navigating medical emergencies, transitions, or conflicts, we provide support to secure the best possible outcomes for our clients.
We provide comprehensive dementia training and education to clients and their loved ones, helping them better understand the disease process and its effects. Our goal is to equip families with the knowledge they need to manage dementia-related challenges, including recognizing signs of caregiver stress and learning effective coping strategies.
We assist clients and their families in navigating the often-challenging transition away from driving. Our team assists our clients and their families with identifying the most appropriate communities and selecting appropriate levels of care such as: independent living, assisted living, or skilled nursing facilities. We also coordinate and assist with moving logistics, including out-of-state transitions, and provide support for a smooth transition into a new home.
We can provide referrals to established elder law attorneys to our clients for assistance with obtaining the legal support they need. Additionally, we are able to provide expert reports to support determinations of care needs during legal proceedings related to care and guardianship matters.
We assist with life care planning of our clients and their families to be prepared before a crisis occurs. This includes discussing and exploring housing and care options, identifying key decision-makers, and reviewing essential paperwork to ensure your wishes are clearly understood and honored.
We provide advisement with completion, reviewing, and organizing long-term care claims and forms. Our goal is to ensure that our clients are maximizing their benefits, minimizing out-of-pocket costs, and navigating the often-complex insurance process.
We understand that life’s challenges don’t adhere to a 9-to-5 schedule, which is why we offer direct access to a Care Manager 24 hours a day, seven days a week. Whether it’s a crisis or an urgent need, our clients can rely on round-the-clock support to address unexpected situations promptly and effectively. This commitment allows our clients to have peace of mind and continuous care whenever it is needed most.
We identify the most suitable services and providers for our clients’ needs; offering referrals to trusted outside professionals such as caregivers, home modification experts, housing options, and support groups. Our recommendations are based on years of industry expertise and experience.
We monitor clients in their homes to observe any changes in condition and identify potential risks, including exploitation or abuse. Our proactive approach ensures that emerging concerns are addressed promptly, protecting the client’s well-being. Additionally, we provide personalized recommendations to enhance security, safety, and comfort through the use of appropriate technologies and other supportive measures.
We coordinate transportation services as part of a comprehensive care plan, ensuring clients have reliable access to medical, social, and other essential appointments and activities. This includes arranging transportation and, when needed, accompanying the client to and from appointments.
